Rent Oh boy, rent in NYC is expensive! It costs me between $30 and $50 per patient depending on how busy my day is.
Laundry I use fresh cotton sheets for each patient, laundered locally with fragrance free detergent. This costs ~$5 per treatment.
Clinic supplies This includes acupuncture needles, moxa, alcohol, cotton balls, ear seeds, cupping oil, headrest covers, sharps boxes, etc. These cost ~ $2 per treatment.
Office supplies Behind the scenes I must buy paper, ink cartridges, business cards, patient files, and a replacement laptop in 2024 when the old one died! That cost ~ $5 per treatment in 2024 (including the laptop!)
License & Insurance I'm fully licensed in NYS and re-register every 3 years. Together with my annual malpractice insurance with AAC, this equates to ~ $3 per treatment. Other costs Booking software, insurance billing, payment fees, city bike, website. ~$3.50 per patient.
That all adds up to $48.5 - $68.5 per treatment. If a patient pays $150, I might get $90. That still sounds like a pretty good hourly rate right? However, that money must also cover time spent preparing for appointments, treatment planning, updating files and other paperwork, booking patients, submitting and dealing with insurance claims, accounting, promotion, etc.
In addition, there is a limit to how many patients I can see in a week to maintain the quality of care I provide, and balance in my own life.

How I calculate my fees I try to align my rates in a logical way that minimizes upfront cost to the patient whether they have insurance or not.
First appointment (1 1/2 hours): 30-40 minute consultation (detailed exam) = $180 + 15 minutes acupuncture @ $75 = $255 or + 30 mins acupuncture = $330
An insurance claim would therefore be for $255or $330. If you are paying out-of-pocket, I charge the lower $255 but you get the longer 30 minute acupuncture!
